Hockey Injuries
Were you/your child involved in a Hockey Injury?
If a patient is injured during a hockey game or practice, they need to print out the Hockey Canada Injury Report. The team safety person has to fill out his/her portion and then bring the form into our clinic and our health professional will fill out the rest. After this is done, our clinic will direct bill to Hockey Manitoba until your injury has been resolved (or up to Hockey Manitoba's maximum cap).
